This Year's R4 PATIENT FUND BENEFICIARY
The R4 Patient Fund 2013 Beneficiary is Johnny Trinidad—known to his friends and family as ‘Trini’.
Trini, 34, lives with his wife Wytza and their two boys, age 1 and 4, in Liberty, N.Y. Trini was diagnosed with Hodgkins disease in 2006. He was treated and remained in remission until January 2013, when he suffered a reoccurrence. In February he had open heart surgery to address lesions in his aorta. He is currently unemployed.
A 9-1-1 vet, Trini served in the Navy Fire Squadron in Norfolk VA from 1998-2003. Long before his diagnosis, he was both an organ and bone marrow donor registrant. Trini is a ‘family man’ – devoted to his children and his large extended family, who live and work in the Sullivan County community.
"R4 PATIENT FUND": The R4 Patient Fund is funded through the sale of raffle tickets. The monies are placed in an account for a designated beneficiary, battling a blood related cancer. The monies are used by local individuals to assist them with the uninsured portion of their medical bills, as well as with uncovered expenses, like fuel, tires, hotel-stays for family members while their loved one is undergoing treatment at a far-away facility.
The United Way of Sullivan County serves as the fiscal agent of the Patient Funds, and distributes the funds to the medical providers or the patient, as needed.
Please show your support for the R4 Patient Fund through the purchase of raffle tickets. They can be purchased by calling 845-794-3600.
The monies raised will assist patients in managing the uninsured portion of their outstanding medical bills. We will raise monies via the sale of raffle tickets, which will be sold the month before and during the event. All money raised from raffles goes directly to the recipient. R4 monies raised through traditional sponsorships, pledges and entry fees are donated to LLS.
Please help with this cause by buying as many raffle tickets as you can.

Individual:
Prizes will be awarded to the Top 3 individuals who raise the most money, through pledges, donations or the sale of raffles.
Teams:
Join with your family, friends and co-workers to form a fund raising team. Teams can consist of five or more
members … the BIGGER the BETTER. Make it fun. Make it competitive. Be creative! Have your team wear identifying shirts, hats, etc. Team members are also fully eligible for all individual prizes.
Be sure to bring all your pledges to Race Day registration. Please convert cash to checks. All checks should be payable to: Rhulen Rock Hill Run & Ramble, Inc.
